Full Salary Range:

Step 1 $3,706.35 Step 2 $3,930.90 Step 3 $4,169.33 Step 4 $4,877.93 Step 5 $4,710.53

Hiring Salary Range: Step 1 $3,706.35/mo. - Step 3 $4,169.33/mo. DOE

Job Summary:

The Case Manager provides outreach, engagement, and intensive case management to individuals referred by law enforcement, EMS, and community organizations. They support individuals facing mental health, substance use, housing instability, or justice system involvement by connecting them to services and resources.

Key Responsibilities:

Conduct outreach and engage participants in the community.
Provide assessments, develop person-centered service plans, and offer long-term case management.
Assist with access to public benefits, housing, and healthcare.
Collaborate with local partners, law enforcement, and service providers.
Offer advocacy in court and support participants through the criminal justice system.
Respond to crises, including after-hours.
Maintain accurate documentation and complete required trainings and reporting.

Required Knowledge, Skills & Abilities:

Strong understanding of harm reduction, motivational interviewing, and case management principles.
Familiarity with mental health/substance use disorders and community resources.
Ability to work independently and within a team under high-stress conditions.
Skilled in advocacy, especially within the legal system.
Cultural competence and ability to support marginalized populations.

Education & Experience:

High school diploma required; bachelor’s degree preferred.
3+ years of related professional experience preferred.
Lived experience with behavioral health or criminal justice involvement is valued.
Street-based outreach experience highly desired.

Other Requirements:

Valid driver’s license.
Must pass background and driving record checks.
Preferred certifications: Washington DOH Agency Affiliated Counselor, NPI.
Must not be excluded from federal healthcare programs.
Must be in stable recovery if applicable.
